Collaborative Ventures Network

Collaborative Ventures Network (CVN), a practice management network, supports health centers in addressing their challenges in today’s health care marketplace. Under Health Resources and Services Administration (HRSA) Health Center Controlled Network (HCCN) Grant H2QCS30263 CVN supports 26 participating health centers (PHCs) (22 in Arizona and four in Nevada), all health center program grantees, in their efforts to leverage health information technology (health IT) for value-based care. Through collaborative efforts, the PHCs endeavor to advance clinical and operational improvement, health IT interoperability, and the use of data to reduce cost and enhance clinical and quality outcomes across 276 total sites (243 permanent, 24 mobile, and 9 seasonal). CVN both assists PHCs in assessing their health IT strategy and development of individualized work plans, as well as, provides support and coaching on all aspects of the HCCN Project. Additionally, CVN facilitates PHC peer-to-peer learning, focus group meetings, and sharing of promising practices. The HCCN Project enables CVN to provide training and technical assistance to PHCs in collaboration with HRSA’s National Cooperative Agreement for health IT, Primary Care Associations, and other partnerships such as the states’ Medicaid Agencies, Health Information Exchanges (HIEs), and academic institutions.
