Community Health Access Network (CHAN)

CHAN is the only HCCN in New Hampshire, with members in Texas and  Vermont, as well. The mission of CHAN is to enable our member agencies to develop the programs and resources necessary to assure access to efficient, effective health care for all clients in our communities, particularly the uninsured, Medicaid, and medically underserved populations and other vulnerable populations. We provide Health Information Technology for our FQHC members including the EMR and Practice Management systems, as well as, a data warehouse through which we provide reporting, registries and dashboards that support clinical quality measurement and improvement, operational and financial reporting. We also provide quality improvement training and technical assistance.

 

CHAN is the project lead for a 3-HCCN collaborative that also includes Breakwater Health Network and Vermont Rural Health Alliance, a program of Bi-State PCA.

 

Breakwater Health Network is a 501©3 horizontal rural Health Center Controlled Network whose Mission is to support its members by developing and sustaining systems that foster clinical excellence and healthy communities. Breakwater Health Network member health centers provide care through 29 clinical sites (including two mobile units) in primarily rural medically underserved areas of Minnesota, Wisconsin, North Dakota, Iowa, and Illinois –– a service delivery area spanning over 100,000 square miles and 118 counties. In addition, the network and its members have several other formal partnerships with other health care and social service entities including public health departments, county health services, critical access hospitals and behavioral health providers. www.breakwaterhealth.org

 

Vermont Rural Health Alliance (VRHA) is a program of Bi-State Primary Care Association​, www.bistatepca.org.   All 11 FQHCs in Vermont are members.

  • A vehicle for health care initiatives; a mechanism to get information back directly to policymakers and state leadership – “Policy into Practice”​
  • Direct connection to environmental drivers ​
  • Alignment of health care initiatives at national and state level; deep understanding of components and how initiatives interlock​
  • Work is directed by the FQHC participants and responds to needs of the FQHCs​
  • Respond to the FQHC regulatory environment
  • Support HRSA goals for FQHCs
