Community Health Access Network (CHAN)

HCCN Description:

CHAN is the only HCCN in New Hampshire. We also have a member in Texas and one in Vermont. We provide Health Information Technology for our FQHC members including the EMR and Practice Management systems, as well as, a data warehouse through which we provide reporting, registries and dashboards that support clinical quality measurement and improvement, operational and financial reporting. We also provide quality improvement training and technical assistance.
